Your jaw and temporal bone are joined by a hinge joint called the
temporomandibular joint, or TMJ. About 25% of the population will experience TMJ pain or dysfunction at some point in their lives.
At Valley Sleep and TMJ we understand the complexities of orofacial pain that are unique to each patient. Some patients experience TMJ(Temporomandibular Joint) pain and dysfunctions (TMD), some experience primary headaches (known as migraines, tension and cluster) while others may have facial neuralgias, musculoskeletal pain or a combination of all. TMJ pain is one of the conditions most frequently identified by patients. This is a very real condition caused by disturbances in the action of the jaw. Research has shown that many factors may lead to this disorder – it can occur by itself or be a part of a more complex problem. There is usually more than one factor responsible. Causes may include macro traumas: automobile accident, sports injury, accidental falls etc. or micro traumas: clenching and grinding of the teeth (bruxism), which causes the muscles that stabilize the joint to become fatigued from overwork resulting in head, face, jaw and muscle pain. Often the jaw problems, head and facial pain, headaches, migraines and musculoskeletal pain are symptoms of a greater problem affecting the body, such as sleep apnea, a condition that deprives the body of oxygen during sleep.
Common symptoms of TMJ Dysfunction
● Headaches – recurring or chronic
● Earache or ear symptoms of stuffiness or ringing
● Neck Pain or Stiffness
● Facial Pain
● Jaw joint sounds – clicking, popping or grating
● Limited ability to open or close mouth
● Jaw locking (opened or closed)
● Sensitive, loose or worn down teeth
● Pain or soreness in the TM joints
● Dizziness or Pain or difficulty chewing or swallowing
● Pain behind the eyes and light sensitivity
Orofacial Pain Services
Proper diagnosis including a detailed patient history and emphasis on origin rather than symptoms alone is key to successful, long term treatment results. After proper diagnosis, a specific non-surgical, treatment plan for rehabilitation is developed for the unique needs of each individual patient.
Orofacial pain services may include one or more of the following:
● Comprehensive diagnostics
● X Ray Imaging, CT and/or MRI imaging to confirm diagnosis
● Non-surgical treatment plan for rehabilitation
● Orthotic therapy (custom stabilizing orthotics-not nightguards or mouthguard)
● Nutritional counseling
● Medication and supplement regimens
● Physical medicine treatment (Laser therapy, trigger point injections, prolo therapy)
● Regenerative PRF injection therapy
● Multi-discipline Referral Coordination (this may include ENT, Physical Therapy, Oral Surgery, Sleep Physician)
Again, proper diagnosis and emphasis on origin rather than symptoms alone is key to successful, long term treatment results. After proper diagnosis, a specific non-surgical, treatment plan for rehabilitation is developed for the unique needs of each individual patient.
PRF Regenerative TMJ Therapy
Dr. Jeffery Sceville is proud to be one of the only providers in the area offering regenerative TMJ therapies using Platelet Rich Fibrin (PRF). Used for years to heal injuries in sports medicine, chronic orthopedic joint problems, and beyond, PRF is transforming how we manage TMJ disorders. PRF works by delivering a concentrated dose of your own platelets and growth factors directly to the source of pain. This advanced treatment helps to:
● Reduce inflammation
● Relieve pain
● Slow or even reverse degenerative joint changes.
What is the process for PRF treatment for the TMJ?
Platelet-Rich Fibrin (PRF) is an advanced treatment that leverages the body's natural healing processes to manage TMJ (temporomandibular joint) disorders. PRF is often used to promote tissue regeneration and reduce inflammation in the joint. Here’s an overview of the process:
1. Patient Evaluation
The doctor will confirm TMJ disorder through clinical evaluation, imaging (e.g., X-rays, MRI), diagnosis and patient history. We then ensure the patient is a good candidate for PRF, considering factors like overall health, medication use (e.g., anticoagulants), and treatment goals. The good news is almost every patient can benefit from PRF because it is 100% natural.
2. Blood Collection
A small amount of the patient’s blood is drawn, typically from a vein in the arm. The volume collected depends on the treatment area but is usually around 10–20 mL.
3. Centrifugation
The blood is placed into a specialized centrifuge and spun at specific speeds to separate its components. This process isolates the fibrin clot, which contains platelets, white blood cells, and growth factors.
4. Preparation of PRF
The platelet rich fibrin cells are carefully removed from the centrifuge tube. It can be used directly depending on the treatment area. Unlike Platelet-Rich Plasma (PRP), PRF requires no additives, as it is 100% natural.
5. Injection or Application
Injection: The PRF is injected directly into the TMJ or surrounding soft tissues. Ultrasound guidance may be used for precise placement. Surgical Use: In cases of surgical TMJ repair, PRF can be applied to the joint or incorporated into grafts to enhance healing.
6. Post-Treatment
Most patient experience little to no discomfort following the PRF injection, however, all our patients are monitored for pain relief, improved mobility, and reduced inflammation. Follow-up treatments may be scheduled, as some patients benefit from multiple sessions.
Benefits of PRF for TMJ
● Anti-inflammatory: Reduces swelling and pain.
● Regeneration: Stimulates the repair of damaged tissues.
● Natural Healing: Uses the patient’s own cells, minimizing risk.
● Long-Lasting Effects: Gradual release of growth factors promotes sustained healing.

It is likely that persons with TMJ who do not receive therapy will eventually develop more serious symptoms. This is due to the fact that the longer someone with this illness goes without treatment, the more likely it is that it will worsen. Long-term TMJ patients who want to have therapy because their symptoms are getting worse frequently require more extensive care and a lengthier course of treatment.
General dentists advise developing a treatment strategy as soon as symptoms start to manifest because doing so will help to control the condition and, ideally, permanently resolve it.
Many people with TMJ pain or dysfunction will have lifelong problems, but with a customized jaw TMJ treatment plan, symptoms may be lessened.
